- Scanned copy of Medical history from the doctor

For applicants who want to submit the physical documents should send:
- Two (2) passport size pictures
- Letter of Acceptance from the university or registration form as certificate of enrollment
- True copy of grades for the entire high school or Transcript of Record
- Medical history and certificate from family doctor
- Filled out application form (downloadable here)
- After submission of the requirements, an appointment can be set for the personal interview with the applicant, accompanied by parent/s.
